  • If you are requesting services remotely or cannot come into our office please submit a Property Owner Utility Services Request .
  • $15.00 new account fee (will be assessed on first bill).

The following is required when requesting utility services with The City of Moses Lake:

  • Complete and submit a Tenant Utility Services Request form (to be completed by tenant) and a Utility Services Billing Waiver form (to be completed your property owner or property manager) via email to [email protected], or in person to the Utility Department, 401 S. Balsam, Moses Lake, WA 98837. Valid government issued photo ID is required.
  • If you are requesting services remotely or cannot come into our office please submit a notarized Tenant Utility Services Request or Utility Services Billing Waiver.
  • The utility account will not be put in your name until both forms are received in our Utility Department. No exceptions.
  • $15.00 new account fee (will be assessed on first bill).

The following is required when requesting a termination of utility services:

  • Complete and submit a Discontinue Utility Services Request in person to the Utility Department, 401 S. Balsam, Moses Lake, WA 98837. Valid government issued photo ID is required.
  • If you are requesting discontinuation of utility services remotely or cannot come into our office please submit a notarized Discontinue Utility Services Request. Please email notarized forms to Utility Billing or fax to (509) 764-3738.
